Title: Preventing congenital neonatal heart block in offspring of mothers with anti-SSA/Ro and SSB/La antibodies: a review of published literature and registered clinical trials.

Summary: Children of women with anti-SSA/Ro - SSB/La antibodies are considered at risk for congenital heart block (CHB). In this literature review, use of hydroxychloroquine (Plaquenil) to prevent CHB is questioned based on lack of evidence for its efficacy and absence of established safety in pregnancy. Two small studies reported superiority of hydroxychloroquine over intravenous gamma globulin (IV-Ig), but a close look in this review revealed significant biases and flawed designs in both studies, leading to the conclusion that both hydroxychloroquine and IV-Ig treatments to prevent CHB in women with anti-SSA/Ro - SSB/La antibodies should still be considered experimental.

Title: Clinical relevance of combined FSH and AMH observations in infertile women.

Summary: Aiming to assess predictive values of different FSH-AMH combinations in IVF cycles, this study investigated 544 infertility patients in their first IVF cycles. The study found that women with both high FSH and high AMH, especially in the absence of het-norm/low FMR1 subgenotype, had significantly more favorable outcomes than women with other combinations of FSH and AMH levels. The finding may suggest a greater importance of FSH in early follicle maturation than is currently appreciated.

Title: Mistaken advocacy against twin pregnancies following IVF.

Summary: Interpreting the data presented in a recent publication by Sazonova et al (Fertil Steril 2013), this review demonstrates that the neonatal and maternal risks associated with twin pregnancies are not "dramatically higher" as claimed by Sazonova et al. Given the minor risk increases with twin pregnancies compraed to singleton pregnancies, the article points out that mandatory single embryo transfers would significantly decrease pregnancy chances without offering compensatory benefits of significantly lower neonatal and maternal risks.

Title: Do hormonal contraception prior to in vitro fertilization (IVF) negatively affects oocyte yields? A pilot study.

Summary: Investigating 43 oocyte donors in 71 IVF cycles, this study found that exposure to high androgenic oral contraceptives prior to IVF cycles suppress functional ovarian reserve and oocyte yields, even in young oocyte donors. The finding of the study suggests that routine use of oral contraceptives in preparation for IVF may require reevaluation, especially in women with diminished ovarian reserve.

Summary: Comparing needles with smaller and larger diameters, this study found no difference in oocyte yields between two needle sizes. Smaller diameter needles significantly prolong time of egg retrieval but were previously shown to decrease pain and bleeding, suggesting that choice of needle for egg retrieval should be individualized.

Summary: Investigating 140 women with premature ovarian aging (POA) and 166 women with age-related diminished ovarian reserve (DOR), this study revealed that DOR is associated with significant hypoandrogenism (excessively low testosterone levels). Hypoandrogenism was especially apparent in younger women. The study raises the question of whether DOR is a phenotypical opposit of PCOS, which often presents with elevated androgen levels.

Summary: This opinion piece sheds light on the often ignored part of medical publishing: the potential conflict of interest in editorial and peer-review process. Pointing out that authors face much more scrutiny than editors and peer reviewers, the article calls for a completely open peer review process to eliminate conflict of interest from medical publishing.

Summary: It has been known for decades that nulliparity is associated with an increased risk for certain reproductive malignancies, including breast, ovarian and uterine cancers. In response to a commentary in The Lancet that related the age-related increase in reproductive cancers in nulliparous women to the number of ovulatory cycles, this commentary postulates other possible primary mechanisms.

Summary: A follow-up to an original pioneering case report from CHR, this study of 21 women confirmed that direct uterine perfusion with G-CSF improves the thickness of uterine lining that did not respond to conventional treatments. After G-CSF perfusions, women in this study expanded their endometrium to approximately 9.3 mm on average, from an inadequate 6.4 mm, enabling them to proceed to embryo transfer. Read more.

Summary: Some IVF patients do not develop adequate endometrium (uterine lining) prior to embryo transfer, even with treatment. In this case study, four patients at two different IVF centers improved their endometrium after receiving intrauterine perfusions of G-CSF, and eventually got pregnant.
