2013
Publication: Autoimmune Rev
Author: Gleicher N, Elkayam U.
Title: Preventing congenital neonatal heart block in offspring of mothers with anti-SSA/Ro and SSB/La antibodies: a review of published literature and registered clinical trials.
Date: 2013
Page #: In press.
Summary: Children of women with anti-SSA/Ro - SSB/La antibodies are considered at risk for congenital heart block (CHB). In this literature review, use of hydroxychloroquine (Plaquenil) to prevent CHB is questioned based on lack of evidence for its efficacy and absence of established safety in pregnancy. Two small studies reported superiority of hydroxychloroquine over intravenous gamma globulin (IV-Ig), but a close look in this review revealed significant biases and flawed designs in both studies, leading to the conclusion that both hydroxychloroquine and IV-Ig treatments to prevent CHB in women with anti-SSA/Ro - SSB/La antibodies should still be considered experimental.
Publication: J Clin Endocrinol Metabol
Author: Gleicher N, et al.
Title: Clinical relevance of combined FSH and AMH observations in infertile women.
Date: 2013
Page #: Epub ahead of print.
Summary: Aiming to assess predictive values of different FSH-AMH combinations in IVF cycles, this study investigated 544 infertility patients in their first IVF cycles. The study found that women with both high FSH and high AMH, especially in the absence of het-norm/low FMR1 subgenotype, had significantly more favorable outcomes than women with other combinations of FSH and AMH levels. The finding may suggest a greater importance of FSH in early follicle maturation than is currently appreciated.
Publication: J Assist Reprod Genet
Author: Gleicher N, Barad DH.
Title: Mistaken advocacy against twin pregnancies following IVF.
Date: 2013
Page #: Epub ahead of print.
Summary: Interpreting the data presented in a recent publication by Sazonova et al (Fertil Steril 2013), this review demonstrates that the neonatal and maternal risks associated with twin pregnancies are not "dramatically higher" as claimed by Sazonova et al. Given the minor risk increases with twin pregnancies compraed to singleton pregnancies, the article points out that mandatory single embryo transfers would significantly decrease pregnancy chances without offering compensatory benefits of significantly lower neonatal and maternal risks.
Publication: Reprod Biol Endocrinol
Author: Barad DH, et al
Title: Do hormonal contraception prior to in vitro fertilization (IVF) negatively affects oocyte yields? A pilot study.
Date: 2013
Page #: Epub ahead of print.
Summary: Investigating 43 oocyte donors in 71 IVF cycles, this study found that exposure to high androgenic oral contraceptives prior to IVF cycles suppress functional ovarian reserve and oocyte yields, even in young oocyte donors. The finding of the study suggests that routine use of oral contraceptives in preparation for IVF may require reevaluation, especially in women with diminished ovarian reserve.
Publication: Reprod Biol Endocrinol
Author: Kushnir VA, et al
Title: A pilot trial of large versus small diameter needles for oocyte retrieval.
Date: 2013
Page #: Epub ahead of print.
Summary: Comparing needles with smaller and larger diameters, this study found no difference in oocyte yields between two needle sizes. Larger diameter needles significantly prolonged time of egg retrieval, suggesting superiority of smaller diameter needles.
Publication: Hum Reprod
Author: Gleicher N, et al
Title: Hypoandrogenism in association with diminished functional ovarian reserve.
Date: 2013
Page #: Epub ahead of print.
Summary: Investigating 140 women with premature ovarian aging (POA) and 166 women with age-related diminished ovarian reserve (DOR), this study revealed that DOR is associated with significant hypoandrogenism (excessively low testosterone levels). Hypoandrogenism was especially apparent in younger women. The study raises the question of whether DOR is a phenotypical opposit of PCOS, which often presents with elevated androgen levels.
Publication: Reprod Biomed Online
Author: Gleicher N
Title: Avoiding currently unavoidable conflicts of interest in medical publishing by transparent peer review.
Date: 2013
Page #: Epub ahead of print.
Summary: This opinion piece sheds light on the often ignored part of medical publishing: the potential conflict of interest in editorial and peer-review process. Pointing out that authors face much more scrutiny than editors and peer reviewers, the article calls for a completely open peer review process to eliminate conflict of interest from medical publishing.
Publication: J Assist Reprod Genet
Author: Gleicher N, et al
Title: Starting and resulting testosterone levels after androgen supplementation determine at all ages in vitro fertilization (IVF) pregnancy rates in women with diminished ovarian reserve (DOR).
Date: 2013
Page #: 30(1):49-62.
Summary: Investigating 213 women with diminished ovarian reserve who were supplemented with DHEA for at least 6 weeks, this study found a correlation between a woman's ability to convert DHEA into testosterone and her pregnancy chances with IVF.
Publication: Reprod Biomed Online
Author: Gleicher N
Title: Why are reproductive cancers more common in nulliparous women?
Date: 2013
Page #: Epub ahead of print.
Summary: It has been known for decades that nulliparity is associated with an increased risk for certain reproductive malignancies, including breast, ovarian and uterine cancers. In response to a commentary in The Lancet that related the age-related increase in reproductive cancers in nulliparous women to the number of ovulatory cycles, this commentary postulates other possible primary mechanisms.
2012
Publication: Hum Reprod
Author: Gleicher N, et al.
Title: A pilot cohort study of granulocyte colony-stimulating factor in the treatment of unresponsive thin endometrium resistant to standard therapies
Date: 2013 (Epub 2012)
Page #: 28(1):172-7.
Summary: A follow-up to an original pioneering case report from CHR, this study of 21 women confirmed that direct uterine perfusion with G-CSF improves the thickness of uterine lining that did not respond to conventional treatments. After G-CSF perfusions, women in this study expanded their endometrium to approximately 9.3 mm on average, from an inadequate 6.4 mm, enabling them to proceed to embryo transfer. Read more.
Publication: J Assist Reprod Genet
Author: Gleicher N and Barad DH.
Title: A review of, and commentary on, the ongoing second clinical introduction of preimplantation genetic screening (PGS) to routine IVF practice
Date: 2012
Page #: 29(11):1159-66.
Summary: Through a review of medical literature and ongoing clinical trials, this study revealed that there is no evidence that preimplantation genetic screening (PGS) improves IVF success rates, despite the recent introduction of new technical abilities. This article is an important warning against "routine" clinical use of PGS for that indication. Read more

Title: Eliminating multiple pregnancies: an appropriate target for government intervention?
Date: 2011
Page #: 23(4):403-6.
Summary: In a commentary following a report on the new government-sponsored intervention into the practice of IVF in Quebec, which included a strict mandate on the number of embryos to transfer, Dr. Gleicher questions the wisdom in such a mandate. This commentary demonstrates that mandated SET (i) is damaging to most fertility patients, because it reduces pregnancy chances; (ii) does not provide compensatory benefit; (iii) impinges on patients' right to self-determination; (iv) negatively affects birth rates.

Author: Barad DH, Weghofer A, Gleicher N.
Title: Utility of age-specific serum anti-Müllerian hormone concentrations.
Date: 2011
Page #: 22;284-291.
Summary: Age-specific levels of anti-Müllerian hormone (AMH) better reflects ovarian reserve than AMH levels that are not age-specific. Compared to age-specific FSH, age-specific AMH defines risk of diminished ovarian reserve or abnormally high oocyte yields (ovarian hyperstimulation syndrome), and therefore may be a particularly useful ovarian reserve test in younger women, in whom diminished ovarian reserve is most frequently overlooked, and who are at highest risk for ovarian hyperstimulation syndrome.
Publication: Reprod Biol Endocrinol
Author: Gleicher N, Weghofer A, Barad DH.
Title: Defining ovarian reserve to better understand ovarian aging.
Date: 2011
Page #: 9:23
Summary: This review article explores the concept of functional ovarian reserve, focusing on the entire pool of follicles available for recruitment and maturation. By looking beyond the gonadtotropin-sensitive phase of folliculogenesis (on which most research has focused so far), we may be able to improve ovarian stimulation protocols for women with diminished ovarian reserve.
Publication: Fertility & Sterility
Author: Gleicher N, Weghofer A, Barad DH.
Title: Successful treatment of unresponsive thin endometrium
Date: 2011
Page #: 95(6):2123.e13-7 [epub ahead of print]
Summary: Some IVF patients do not develop adequate endometrium (uterine lining) prior to embryo transfer, even with treatment. In this case study, four patients at two different IVF centers improved their endometrium after receiving intrauterine perfusions of G-CSF, and eventually got pregnant. This study suggests that G-CSF may be a promising tool to improve implantation. Read the details.

Publication: Autoimmunity Reviews
Author: Gleicher N, Weghofer A, Barad DH.
Title: Do chromosomally abnormal pregnancies really preclude autoimmune etiologies of spontaneous miscarriages?
Date: 2011 (Epub in 2010)
Page #: 10(6):361-3.
Summary: Current dogma in both rheumatology and obstetrics/gynecology holds that evidence of aneuploidy in miscarriages rules out autoimmune etiology. However, literature review suggests that maternal autoimmunity itself may increase the risk toward aneuploidy in abortuses and offspring. Read the details.
