Egg Donor Application Policies and Procedures

Authorize Consultation

In order to authorize our infertility specialists to review your Medical History, you must complete Step 2, The Authorization Form. The Complete Consultation is $350 and covers our costs to provide you an expert to review your Medical History. Once you complete the Authorization Form, our infertility specialists will review and e-mail you our recommendations within 5 business days. The Complete Consultation with a follow-up Telephone Consultation is $475, and the Complete Consultation with an Office Visit is $600.
