How to Complete & Submit the New Patient Questionnaire

Welcome to the Center for Human Reproduction. At CHR, we ask you to complete our New Patient Questionnaire before your initial appointment with our physicians. If you haven't set up an appointment yet, please call us at 212-994-4400 first before proceeding. If you are ready to fill out the questionnaire, please click here to go to the New Patient Questionnaire page.

When you click the "Submit" button at the end of the questionnaire, your answers will be sent to CHR. This way, your physician can review your medical history and be ready for the appointment with you.
