With conflicting information offered on the Internet, and varying treatments as well as levels of expertise offered by fertility centers, patients often find it difficult to conclude whether their current treatment plan is the right one for them. With convenient email-based infertility second opinion consultations, CHR's fertility experts can help you navigate through your fertility diagnoses and treatment options.
Second opinion consultation starts with Step 1: submitting the second opinion consultation form, and Step 2: authorizing consultation wtih a credit card. You will receive an email second opinion from one of our experts within five (5) days. Please see below for more details of each step.

After a second opinion consultation, you may decide to pursue a different path in your fertility treatment, or make modifications to your current plans. Or you may be reassured that your current treatment path is the best one available for your specific situation. It is always good to feel informed and confident about your fertility treatment decisions, and we are here to help you reach this point via an fertility expert second opinion consultation.
Step 1: Complete Second Opinion Consultation Form
The first step in the second opinion consultation process is to complete a confidential questionnaire about your family history, personal health history, medications, child bearing history, your attempts at pregnancy, and outcomes of prior fertility treatments.
Please try to be as accurate and thorough as possible. The more information you can give us, the better we will be able to assess your situation. If you have laboratory results, please include them in the consultatoin form. We are especially interested in your FSH, estradiol and AMH levels. You can also fax this information to 1 212-994-4400. Please make sure to mention your name and "second opinion consultatoin" in the cover sheet.
Step 2: Authorize Consultation
After completing the second opinion consultation form, please proceed to the step 2, authorization of payment. Upon receiving your authorization of payment, our receptionist will contact you to confirm your second opinion consultation. The cost of a complete email-based second opinion is $350. If your case requires a follow-up consultation, the costs are as follows:

Analysis
Our fertility specialists will thoroughly review your medical history. If we require more information than you submitted, we will be in touch. If you have additional information for us, such as recent test results, you can fax the information to 1 212-994-4499.
Recommendations
Based on the information we receive from you, we will provide our second opinion within five (5) business days. If, for some reason, we are delayed in our response, we will contact you with an update on when you can expect to receive our second opinion. Our second opinion will be provided to you in an email for your convenience.
Your options afterward, if you require a change of course in your treatment plans, may include a follow up consultation with one of our physicians, either by phone or in our Manhattan office, or a referral to an appropriate fertility specialist in your area.
If you decide to pursue further fertility treatments at CHR after your second opinion consultation, we will discount the cost of this second consultation from your treatment costs.

Our second opinion is based exclusively on the information transmitted to us electronically. The second opinion, therefore, cannot be equated to a full medical consultation which relies on a face-to-face encounter between a physician and a patient, and, where indicated, physical examinations and tests can be performed.

Patients submitting a request for a second opinion to CHR should, therefore, act upon the opinion only after having a personal consultation with a physician.
