How much does the Second Opinion Consultation cost?
We charge $350 for a Second Opinion Consultation.

long will I have to wait for recommendation?
The Second Opinion Consultation process, after we receive your authorization,
takes up to 5 business days.

| 3) | Is Second Opinion covered by insurance?
We do not accept insurance assignments for the Second Opinion Consultation. However,
many insurance companies encourage Second Opinions, and therefore, reinburse
costs for Second Opinions. We will, free of charge, fill out claim forms
for insurance if within 14 days from the initial consultation, they are
sent to our administrative office at 650 West Lake Street, Chicago, IL
60661.

Will I be able to apply this consultation to other services?
If you sign up for a Second Opinion and, within one month, request additional
services from CHR, we will discount the new services by the cost of the
Second Opinion.

Who is giving the Second Opinion?
Our infertility specialists will
provide the analysis and recommendations contained in the Second Opinion in written
format.
