Early Retrieval Triples Pregnancy Chances for POA Patients

Retrieving eggs early in an IVF cycle vastly improves egg/embryo quality and IVF pregnancy rates for young women with diminished ovarian reserve (DOR), according to a new study from CHR, a fertility center in New York City. Dr. Norbert Gleicher, CHR’s Medical Director, explains how the early retrieval protocol, developed at CHR, works, and why retrieving eggs early when their follicles are still 14-18 mm in size, might improve pregnancy chances DOR.

In a paper CHR investigators published in 2015 in a prestigious endocrinology journal, we demonstrated that in older women, and by that I mean women with mean age of 44 and with all being above age 43, in those women, early egg retrieval doubled pregnancy chances.

For those women who have the ability to still produce eggs and embryos, it demonstrates that we are steadily improving outcomes, and it’s worthwhile giving them a chance to try to conceive with their own eggs, rather than sending them right away to egg donation.

Recent research at CHR has in fact confirmed that women with POA see exactly the same molecular profile developing inside their ovaries and their follicles as we previously observed in much older women above age 43.

Like older women above age 43, women with POA demonstrate molecular evidence for what we call premature luteinization, meaning that follicles mature quicker, and eggs in these follicles mature quicker in parallel. This also means that eggs become over-mature, overcooked in those follicles.

The same treatment that we previously found effective in older women, namely, removing the eggs early from those prematurely maturing follicles, is equally, actually more, effective in younger women than in older women. In older women, we only doubled pregnancy chances, but in younger women, we have seen a tripling of pregnancy chances. We are looking forward to being even more successful in our outcomes in treating women with premature ovarian aging.
