CHR has contractual agreements with the following insurance companies and PPO networks. We encourage you to contact your benefit administrators to understand the covered services within your plan. If your plan is not currently listed or does not have infertility coverage, you have a few options:

*** Our contract with GHI terminated on September 1st, 2011. After many years of a highly satisfactory relationship, recent policy changes in regards to infertility services at GHI, combined with a reimbursement schedule, which has not kept up with increasing costs in providing state-of-the-arts infertility services, led us to the difficult decision to cancel our contract with GHI as of September 1, 2011. This means that, as of that date, CHR no longer "accepts" GHI insurance coverage, and patients insured under all GHI Healthplans are required to make cash payments in order to receive services at CHR.
